yes, but if you happen to forget or not be in a place where that is possible, the dentist always has toothbrushes and toothpaste AND mouthwash available at a sink for you to use before you see the dentist. don't be afraid or too shy to ask.

Absolutely. I even take a toothbrush to work to do it if I am not going to have time to go home before my appointment.
Yes. More aggressively than normal, too.
I don't. I'll chew gum to take care of any bad breath, but I don't brush again (after my normal morning brushing) before the appointment.

of course. And I start flossing about a week before as well.
Except once I hit the late 30s and, eek 40, I floss sporadically, too.
